What is ER about?

ER ran for fifteen seasons on NBC, which is itself a kind of argument. No drama sustains that kind of run on sheer momentum. Michael Crichton created it, Steven Spielberg produced it, and when it premiered in 1994 it genuinely changed what network television thought a medical drama could look like: fast, loud, technically specific, emotionally punishing. The show follows the staff of County General Hospital in Chicago, and at its best it makes you feel the actual weight of a twelve-hour shift, the way competence and exhaustion and moral compromise get all tangled up together.

The tone is relentless in a way that became the template for almost every procedural drama that followed. The long Steadicam takes through crowded corridors, the acronyms fired like weapons, the patients who arrive as problems and leave as people, or don't leave at all. What made early ER distinctive was its refusal to soften the medicine or the psychology. Characters made bad decisions, sometimes fatal ones, and the show let those decisions sit there.

Critics adored it in its first several seasons, and audiences came in enormous numbers, making it one of the highest-rated dramas in television history during its peak. It collected Emmy nominations the way some shows collect modest renewals. The later seasons, particularly the ones represented by the cast listed here, were received more warmly by loyal fans than by critics, who generally felt the show had outlasted its original creative energy. That is probably fair. Angela Bassett brought genuine authority to her role, and Maura Tierney had always been one of the show's more undervalued assets, but the final years had the feeling of a great restaurant coasting on its reputation.

What is worth remembering is that ER essentially trained a generation of viewers to expect more from hospital drama, and then had to compete with the shows that learned from it. The show didn't get cancelled; it concluded on its own terms, and the ending felt earned rather than abrupt. By the time NBC and the producers agreed to wrap things up, the show had already outlasted most of its original cast and had gone through more reinventions than almost any drama in television history. George Clooney had long since become a movie star. Anthony Edwards, Julianna Margulies, and others had moved on years earlier. The show kept finding new doctors and new stories, but the audience had thinned considerably from the peak years in the late 1990s, when ER was the most watched drama on American television.

The ratings decline was gradual rather than catastrophic, which is partly why NBC kept it going as long as they did. At its height the show drew over 30 million viewers a week; by its final seasons that number had dropped to something closer to 10 million, which is still respectable but reflects how much the television landscape had changed. Cable drama had matured, viewers had more choices, and the procedural medical genre had been crowded out by newer entries like Grey's Anatomy. ER was no longer the only game in town, or even the dominant one.

What the finale managed to do, though, was close things out with a sense of history. The producers brought back some original cast members, and the final episode leaned into the show's long legacy in a way that felt like a genuine goodbye rather than a last gasp. Shows rarely get to end cleanly after fifteen years. ER did.
